Business Records Management (BRM) is the records repository of the medical records for the Late Dr. Lawrence Dunegen's physician practice. The medical records will be indexed and stored in our secure facilities. All records will be destroyed on May 1, 2022.
Requesting Medical Records
Patients in need of their medical records can request their individual records by completing a Release of Information form. The Release of Information form may be downloaded via this website. Once the form is completed, it must be mailed, along with the search and retrieval payment to:

Payment Information
BRM will charge a flat fee of $19.80 for the search and retrieval (which is non-refundable). Upon receipt of the Release of Information Form and payment of $19.80, BRM will invoice you for the per page photocopy cost and postage. These fees are in accordance with
Money orders, cashiers checks and personal checks will be accepted as payment.
Availability of Released Medical Records
Fulfilled requests will be distributed within seven (7) days after the photocopy and postage invoice is received at BRM.
